Request for Monitoring and Clearance

All requests shall be coursed through the online request slip portal

The Foundation may send an email to cgfd_md2@sec.gov.ph requesting for monitoring and indicating the purpose of its request. The email processor will then list all the documentary requirements the Foundation needs to submit and/or request the Foundation to fill out the request slip request slip above. The documents differ depending on the purpose of the request for monitoring.
The following format shall be used in the subject head: CGFD_NAME OF COMPANY_REQUEST FOR MONITORING_DATE OF EMAIL
Walk-in
The Foundation, through its authorized representative, may visit our office at Ground Floor, North Wing Hall, Secretariat Building, PICC Complex, Vicente Sotto Street, 1307 Pasay City.
The staff on skeletal duty will then request the Foundation to scan a QR Code posted in the office which will direct the Foundation to the request slip portal.

Summary of the Request for Monitoring Process

STEPS APPLICANT/CLIENT SERVICE PROVIDER
1 Accomplish request slip for monitoring. Officer of the Day assigns to handling processor/staff.
2 Present documentary requirements. Check if documentary requirements presented are complete and correct.

Wait for the result of request.

I. If compliant, wait for issuance of MS and CIS Clearance. End of Transaction.

II. If not compliant, wait for issuance of MS.

Processor/Staff monitors the company’s compliance with SEC reportorial requirements. Supervising Specialist/ Assistant Director reviews Monitoring Sheet (MS).

I. If compliant, issue MS and generate Clearance via the Certification Issuance System (CIS). End of Transaction

II. If not compliant, issue MS

Note: Requests made in connection with an amendment of Articles of Incorporation and/or By-laws must be forwarded to a Legal Counsel for review.

If with assessed monetary penalty, request for Payment Assessment Form (PAF) and pay at the cashier.

Cashiers at the SEC Main Office and Ortigas Office – SEC PAYMENT FACILITIES

SEC Online Collection Payment/Deposit thru Landbank Branches (as indicated in the PAF)

For National Capital Region (NCR), payments are allowed only thru the following branches:

Araneta E.O.
YMCA
DOTC
ORTIGAS E.O.
Muntinlupa
North Avenue

SEC Payment Portal using digital wallet or credit card (https://espaysec.sec.gov.ph/payment-portal/home)

If with assessed monetary penalty, generate PAF and advise applicant/client to pay at the cashier.
5 Present copies (original and photocopy) of the PAF and Official Receipt (OR). Wait for issuance of CIS Clearance. End of Transaction Retrieve copies of the PAF and OR. Generate Clearance via the CIS. End of Transaction

Submission of Documents
The hard copies of reports, letters, requests and other documents of Foundations, Accredited Microfinance NGOs, Lending Companies and Financing Companies, shall be filed through the following means:
Via Courier Service only – Printed or hard copies of the following documents of covered companies with wet signature and proper notarization should be sent through courier or Philippine Postal Corp. using the SEC Express Nationwide Submission (SENS) facility https://sens.secexpress.ph/.
Printed or hard copies of documents shall be deemed to have been filed on the date they were received by the courier.
Via email only – Scanned copies of the printed or hard copies of following documents with wet signature and with proper notarization as necessary may be sent in Portable Document Format (PDF) to the following address: cgfd_md2@sec.gov.ph
Via Online Submission Tool (OST) – The Online Submission Tool (OST) is an online facility to be used for submitting the Audited Financial Statement (AFS), General Information Sheet (GIS), Sworn Statement for Foundations (SSF), General Form for Financial Statements (GFFS), Special Form for Financial Statement (SFFS) and other reportorial requirements. All corporations registered with SEC must enroll in the OST in order to access and submit reports through the system. https://cifss-ost.sec.gov.ph/user/login
